Make a significant and meaningful difference to people’s lives in an Aboriginal Community Controlled Health Service (ACCHS), that represents the health needs of Aboriginal and Torres Strait Islander people across the Northwest, and Lower Gulf regions of Queensland. This ACCHS is committed to supporting Aboriginal communities’ own solutions to living long healthy lives; strengthen their culture; and regain their spirit. Also, this ACCHS provides a broad range of primary health care and aged care services and is now the largest Aboriginal Community Controlled Health Service provider by land area in Queensland.

The Aged Care Connector (ACC) has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements.

The overarching function of the AAC is to ensure that support for clients is organised, sequenced, and supported to enable effective access to appropriate aged care services both internal and external to the ACCHO.

Working closely with the Aged Care Support Coordinator, the ACC will help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.

Qualifications & Experience:

• Qualifications in Community Services and/or Aboriginal Mental Health First Aid is highly desirable.

• No specific experience required; training will be provided.

Skills & Capabilities:

• Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Community.

• Good communication and interpersonal skills, with the ability to build trusting relationships in community.

• Demonstrated understanding of the principles of person-centred care and trauma-informed practice.

• Experience working with older people.

• Knowledge of or willingness to learn about the aged care landscape.

• Ability to or willingness to learn how to enter data and reporting.

• Demonstrated experience working unsupervised and as part of a team.

Other Requirements:

• Current C Class Driver’s License (QLD) – essential.

• Satisfactory police check – no serious Criminal/Court record.

• Current Blue Card (working with children and young people) or willingness to apply.

• Willingness to work flexible hours as required, including the requirement to travel, work and stay overnight for up to several days at a time in remote areas.
